Stress & Depression Screening Questionnaire

We encourage all UC San Diego Health team members and students to complete this brief online stress and depression screening. After completing this anonymous and confidential questionnaire, one of our experienced counselors will send you an assessment with recommendations for further evaluations or follow-up.

HEAR is a unique support service, but it is not considered a psychiatric treatment or crisis/emergency intervention.

If you or someone you know is in crisis, please call or text the crisis helpline at 988 or send a chat on 988lifeline.org.

How Can HEAR Help You?
